CCSNH has partnered with NextGen Web Solutions to help students submit their financial aid forms online via Dynamic Forms. Beginning with the 2021-22 financial aid year, students at LRCC, MCC, NCC, NHTI, RVCC and WMCC will now be able to securely upload information to their College’s Financial Aid Office and electronically sign forms. GBCC is not currently utilizing Dynamic Forms.

Accepted & Current Students

Any required financial aid forms will be listed in the Eligibility section of the Student Information System (SIS). Click on the requirement to automatically be brought to the Dynamic Forms system. You will be presented with the CCSNH login page. Login with your CCSNH username and password to complete the form.

Prospective and new students who do not yet have a CCSNH username and password must create a Dynamic Forms account in order to submit financial aid information and signatures electronically. Click here to view instructions on how to create your account.

All financial aid forms must be initiated by the student. The student will enter their parent’s or spouse’s email address into the form. The parent/spouse will receive an email containing a link to the form. Before submitting information or electronically signing a form, the parent/spouse must create a Dynamic Forms account. Forms Available to Accepted & Current Students

  1. Accepted and current students must log into Dynamic Forms using their CCSNH username and password.
  2. Click on the form name below and then your College’s link to complete the required form. Note: At this time, GBCC is not utilizing electronic forms.
  3. Any supporting documents uploaded with a form must be in PDF format.

Accepted & Current Students

Students will access Dynamic Forms through the Eligibility section of Student Information System (SIS ) by clicking on the links within the required document list. After clicking on the link in SIS, students will be presented with the CCSNH login for Dynamic Forms. Using their CCSNH username and password, students will login to the specific Dynamic Form. Any documents (e.g. tax transcripts, W-2s, etc.) uploaded to Dynamic Forms must be in PDF format.

You need to log back into Dynamic Forms using your CCSNH username and password, click on My Forms and then Forms History to review what forms you have submitted.

You need to log back into Dynamic Forms using your CCSNH username and password, click on My Forms and then Pending/Draft Forms to review/submit  forms you have outstanding.

You need to log back into Dynamic Forms using your CCSNH username and password, click on My Forms and Pending/Draft Forms. On the form you need to update, click Action and then Manage Cosigners.

Prospective Students, Parents & Spouses of Students

You need to log back into Dynamic Forms and then click on Forms History to view what forms you have submitted.

You need to log back into Dynamic Forms and then click on Pending/Draft Forms to view any forms that are incomplete.
